Baptist Memorial Rehabilitation Hospital is a 53 bed in patient acute rehabilitation hospital located in Germantown, Tennessee and is part of Lifepoint Health, a diversified healthcare delivery network committed to making communities healthier® with acute care, rehabilitation, and behavioral health facilities from coast to coast. Requirements

  • Minimum of 1 year of experience in hospital or post-acute healthcare setting.

Nice To Haves

  • Current college degree in healthcare-related field preferred
  • Preference for case management or clinical department support experience.
  • LPN or Social Work degree preferred

Responsibilities

  • Support case managers with tasks such as preparing referral packets, organizing admission and discharge records, making calls, and obtaining signatures.
  • Assist in arranging post-discharge follow-up care and services.
  • Submit necessary clinical documentation for insurance reviews (admission, concurrent, retrospective).
  • Monitor and track payer responses including approvals, appeals, denials, and communicate updates to the appropriate internal teams.
  • Track certified hospital days and notify case management staff of missing certifications.
  • Perform other related duties as assigned.
