About The Position

Visit patients in various hospital units @ St. Luke’s before their discharge Schedule follow-up appointments and establish new primary care providers for patients Complete and add messages to Medicare forms Occasional audits of Medicare forms Collaborate and communicate with RN Case Managers Answers, screens and initiates telephone calls, triages clinical calls and resolves problems appropriately

Requirements

  • High School Graduate.
  • 1 year of experience in performing scheduling, customer service, and administrative/clerical support duties.

  • Visit patients in various hospital units @ St. Luke’s before their discharge
  • Schedule follow-up appointments and establish new primary care providers for patients
  • Complete and add messages to Medicare forms
  • Occasional audits of Medicare forms
  • Collaborate and communicate with RN Case Managers
  • Answers, screens and initiates telephone calls, triages clinical calls and resolves problems appropriately
