Requirements

• Current Registered Nurse, Physical Therapist, Occupational Therapist, Speech Language Pathologist or Social Work degree.
• Minimum of 1 year of experience in hospital or post-acute healthcare setting.
• Proficiency in Microsoft Word, Excel, keyboard typing, and use of general office equipment.

Responsibilities

• Provide administrative and clinical support to the case management team.
• Ensure coordination of post-discharge services.
• Facilitate payer communication and documentation tracking.
• Help coordinate family trainings and follow-up appointments.
• Assist in effective patient transitions.
