About The Position

As a Case Management Assistant PRN with HCA Florida West Hospital, you will be part of an organization devoted to giving back to the community. The role involves assisting with facilitating patient discharges requiring post-acute placement or services, and providing administrative support in Case Management functions. You will work under the direction of the RN case manager or social worker, coordinating with them to ensure timely patient care and discharge processes.

Requirements

  • Associate Degree Preferred.
  • Medical knowledge and terminology Required.
  • Minimum of one year experience in a health care setting Required.

Responsibilities

  • Assist with facilitating patient discharges requiring post-acute placement or services.
  • Identify and report barriers to discharges.
  • Provide administrative support in Case Management functions.
  • Coordinate with RN case manager and social worker for support needed.
  • Communicate updates on progress throughout the day.
  • Refer patient questions back to the case manager/social worker.
  • Provide clerical support in multidisciplinary rounds.
  • Make referrals through the web-based referral system to specific post-acute care providers.
  • Monitor patients with PAC referrals and intervene to expedite discharge.
  • Prioritize workload to ensure timely contact and intervention with PAC providers.
  • Communicate with RN case managers/social workers regarding the status of PAC referrals.
  • Deliver the Important Medicare Message to Medicare patients.
  • Obtain physician signature on required Case Management documents.
  • Set up transportation and generate discharge packets for PAC transfers.
  • Maintain knowledge of and develop relationships with PAC providers.
  • Adhere to regulatory requirements and HCA Ethics and Compliance policies.
