Job Summary Responsible for overseeing the daily operations of clinical care coordination and case management functions. Ensures the delivery of high-quality patient care by optimizing workflows, managing staff performance, and facilitating communication between care teams. Works to streamline processes, improve patient outcomes, and ensure compliance with hospital standards and regulations.

  • Bachelor's Degree Nursing required or Bachelor's Degree Healthcare Administration required or Bachelor's Degree Related Field of Study required
  • Registered Nurse [RN - Massachusetts]
  • Clinical experience in case management or care coordination, 3-5 years required
  • Experience in a supervisory role, 1-2 years required
  • Knowledge of healthcare regulations, care coordination best practices, and patient care standards is required.
  • Strong leadership and team management skills to effectively guide clinical staff.
  • Excellent communication and interpersonal skills for collaborating with healthcare teams and patients.
  • Analytical skills to monitor and improve clinical operations and patient outcomes.
  • Problem-solving abilities to address challenges in care coordination and operational workflows.
  • Proficiency in healthcare technology and case management systems to optimize workflow efficiency.
