About The Position

This role is part of Aetna’s Long-Term Services and Supports (LTSS) program, seeking a member-focused Case Management Analyst to support individuals and families in the Northern Virginia region. This is a field-based position involving significant face-to-face interaction with members to listen, advocate, and help them navigate complex health and social needs. Requirements

  • 2+ years of experience in behavioral health, social services, or a related field aligned with program focus.
  • Residency in one of the following Virginia areas: Alexandria, Arlington, Vienna, McLean, Woodbridge, and/or Springfield.
  • Reliable transportation and the ability to travel up to 70% of the time (mileage reimbursed per company policy).

Nice To Haves

  • Experience in case management or discharge planning.
  • Managed Care experience.

Responsibilities

  • Build trusted relationships with members through in‑person visits (approximately 70% travel).
  • Conduct comprehensive assessments using clinical judgment and critical thinking to understand medical, behavioral, and social needs.
  • Develop and implement individualized care plans that promote health, independence, and quality of life.
  • Collaborate closely with providers, community resources, and interdisciplinary teams to ensure coordinated, high‑quality care.
  • Connect members to services, supports, and resources that align with their goals.
  • Ensure care delivery meets regulatory requirements and Aetna quality standards.
  • Advocate for medical appropriateness while supporting positive member engagement and outcomes.
