Requirements

  • Clinical background required, including a nursing degree or equivalent clinical education, or relevant clinical experience consistent with Case Development requirements.
  • All Case Development staff must maintain readiness to be deployed, at LiveOnNY’s discretion, to perform hospital-based services in support of donor evaluation, donor management, continuity of care, surge response, or operational needs.
  • The role may involve off-shift work and requires proficiency in independent, self-directed task execution while collaborating virtually and in-person with other LiveOnNY personnel and donor hospital medical care and administrative personnel.
  • All work assignments must remain aligned with LiveOnNY policies and operational needs.
  • Bachelor’s degree is preferred.
  • CDC I -Clinical background required, including a nursing degree or equivalent clinical education, or a minimum of one (1) year of experience in emergency department, critical care, ICU, transplant services, or organ procurement organization coordination. Requires the ability to apply clinical judgment in a high-volume, contact center–based environment.
  • CDC II - Minimum of one (1) year successful performance in the Case Development Coordinator I (CDC I) role (or equivalent experience as determined by LiveOnNY). Demonstrated ability to apply clinical judgment within defined protocols with increased independence. Clinical background required, including a nursing degree or equivalent clinical education, or relevant clinical experience consistent with Case Development requirements.
  • CDC III - Minimum of two (2) years successful performance in the Case Development Coordinator II (CDC II) role (or equivalent experience as determined by LiveOnNY). Clinical background required, including a nursing degree or equivalent clinical education, or relevant clinical experience consistent with Case Development requirements.

Responsibilities

  • Receive, review, and process potential donor referrals and related calls from donor hospitals.
  • Perform structured clinical review of referral information to assess donor potential using established criteria.
  • Enter and maintain referral and donor evaluation information in the electronic donor record (EDR) and related systems.
  • Identify when predefined clinical or authorization indicators are met and escalate cases for further review or action.
  • Serve as a consistent source of qualified information and guidance during the assigned shift to support continuity of donor evaluation and management.
  • Identify case-specific risks or barriers to donation potential and escalate concerns in accordance with LiveOnNY protocols.
  • Provide timely feedback to donor hospitals regarding referral and/or potential donor evaluation and management status and next steps in accordance with SOPs.
  • Coordinate timely notification and escalation to Family Engagement, Hospital Services, Allocation and Placement, Clinical Leadership, Medical Director, or other subject matter experts when cases progress or require additional review.
  • Utilize LiveOnNY call center platforms, telephony systems, CRM tools, electronic donor records, and reporting dashboards to manage a high-volume, call-driven workflow involving continuous incoming and outgoing referral calls and documentation.
  • Manage multiple simultaneous referral calls and donor evaluation and management calls (incoming/outgoing) activities while maintaining accuracy, professionalism, and regulatory compliance.
  • Ensure accurate, timely documentation of all referral and potential donor case evaluation and management activity in accordance with LiveOnNY policy and regulatory requirements.
  • Identify, document, and escalate process improvement opportunities to reduce missed donation opportunities and strengthen consistency.
