About The Position

We are Aspire Living & Learning, a non-profit human services agency making a meaningful difference in the lives of adults and children with autism and other disabilities. Aspire Living & Learning is currently seeking a Case Coordinator to be responsible for the management of cases under the supervision of a licensed clinical provider or department head. The Case Coordinator will work collaboratively with all internal and external team members to offer support where needed. Schedule: M-F 8a-4p Location: Hybrid, Leominster, MA Pay: $20-$23 hourly, based on education and experience

Requirements

  • High school diploma or GED required, bachelor's preferred
  • Must meet the Registered Behavior Technician requirements
  • Excellent interpersonal, organizational, and communication skills
  • Computer skills
  • Must have satisfactory background checks
  • Valid driver’s license and safe driving record

Responsibilities

  • The Case Coordinator carries a caseload assigned by the supervisor, director, or department head.
  • Demonstrates the skills to work with a variety of individuals of varying ages and functioning level.
  • Dependent on service line, develops, oversees implementation of behavior, treatment, educational, or medication support plans, as applicable.
  • Assists in the modeling and teaching of behavioral or mental health strategies to necessary stakeholders (e.g., staff, providers).
  • Trains the managers, DSP leads, etc. on any plans, or individual specific training, when applicable.
  • Monitors, verifies, and documents various levels of programmatic or clinical support or service provision.
  • Completes all required documentation.
  • Meets deadlines.
  • Ensures relevant sections of electronic record is up to date and accurate.
  • Facilitates or attends all team meetings.
  • Effectively communicates in both verbal and written form with necessary stakeholders.
  • Accepts feedback from supervisors to ensure the quality of program implementation.
  • Communicates and collaborates with both internal and external team members and stakeholders.
  • Maintains ethical and professional boundaries and guidelines.
  • Works cooperatively and collaboratively with all necessary stakeholders.
  • Cultivate a positive and productive workplace atmosphere to enhance team morale and productivity.
  • Participate in and complete training sessions as delegated by management to enhance skills and knowledge relevant to the role.
  • Conduct training as required and assigned.
  • Perform any additional job responsibilities assigned by supervisors or team leaders to support the organization's smooth operation.
  • Accept reassignment as required.
