About The Position

Case coordination assists individuals in achieving physical and mental wellness through the planning of health and social services. This includes, but is not limited to, advocacy, assessment, planning, and education. The case coordinator, along with the care team, will ensure that the individual is established with services in a coordinated, effective, and efficient manner. The case coordinator will offer support as well as monitor and track an individual's progress throughout the course of his/her treatment.

Requirements

  • High School Diploma or equivalent required.

Nice To Haves

  • 1 year in a drug and alcohol or mental health setting preferred.
  • Driver’s License preferred.
  • CPR/BLS preferred.

Responsibilities

  • Coordination of client engagement through appointments, aftercare plans, schedules, and meetings in collaboration with the treatment team.
  • Coordination and communication with referral sources and outside providers through consistent, professional communication, assessment of needs, and documentation of contact at admission and for the duration of treatment. This may include communication or coordination for UR, legal, and employment needs for the client.
  • Provides case coordination services for clients through coordinating transportation needs.
  • Providing individual case coordination sessions.
  • Assists in the development of aftercare plans, providing referral and aftercare provider contact, and addressing outside provider needs.
  • Assists with groups to support clinical services provided and supports clients on a weekly basis to encourage engagement in the program.
  • Participates in treatment team collaboration and maintains professional, collaborative communication with counselors and the treatment team.
  • Documentation of client, family, referral, and outside provider contacts in Carelogic in accordance with program timelines.
  • Collaboration with the client navigation team for client aftercare needs.
  • Provides onsite support for Enhanced Outpatient Sessions (EOS) appointments between clients and the Client Navigation team.
  • Participates in mandatory corporate training, program training, and professional development.
  • Other duties as assigned.
