About The Position

This position will require travel for assessments RESPONSIBILITIES Complete the required QSAC orientation and trainings. Ensure health, safety, welfare of individuals. Maintain correspondence with parents and other providers. Maintain individual/family confidentiality. Attendance and punctuality are essential. Commitment to company values and adherence to policies. Establish protocols to facilitate CAS Assessments in a timely manner. Coordinate with the Nursing and Clinical departments to obtain medical, behavioral, and psychiatric records prior to the CAS Assessment. Coordinate with the Residential and/or Day Habilitation departments to obtain historical and service records prior to the CAS Assessment. Sit in on all interviews related to the CAS Assessment. Coordinate scheduling for all CAS Assessments between the program(s), the family, and the CCO. Ensure all required documentation is reviewed and submitted in a timely manner. Responsible for training staff and family members in the CAS Assessment process. Perform other duties as assigned by supervisors and/or senior management.

Requirements

  • Bachelors Degree and substantial related experience.
  • Ability to safely assist in lifting individuals of various weights & 20 lb. items.
  • Must be able to communicate effectively with others and individuals served.
  • Clearance through state mandated Background/Fingerprint Check(s).
