About The Position

Humboldt Senior Resource Center is looking for a CarePartner to support our participants in our Adult Day Health Services program. The schedule for this position is Monday through Friday, 8:15 am to 3 pm. Adult Day Health Care is a therapeutic day program that provides care to enhance the independence and quality of life for adults age 18 and older with physical and behavioral health conditions, and provides respite for family caregivers. Our program includes: Nursing supervision of medications, medical care, and treatments; social work case management; physical, occupational, speech, and nutritional therapies; individual and group exercise; behavioral health services; personal care services; nutritious snacks and lunch; recreational activities and special events; and round-trip transportation from their home. JOB SUMMARY: Provide therapeutic services and personal care to program participants while assisting with daily activities that promote health, independence, and social engagement. CarePartners are integral members of the Multidisciplinary Team, collaborating with Adult Day Health staff and other programs to provide quality, person-centered care.

Requirements

  • Must possess a high school diploma or GED equivalent.
  • Must have at least one (1) year of experience working with a frail or elderly population, preferably providing personal care in a health facility or in-home setting.
  • Current CPR and first aid certification within six (6) months of hire.
  • This position requires the successful completion of a background check.
  • Pre-employment physical, and PPD skin test (TB screening).
  • Demonstrates an interest in working with elderly and disabled adults to enhance and encourage their independence.
  • Basic clinical knowledge with an understanding of hygiene, skin care, and vital signs.
  • Able to safely support transfers, ambulation, and use of mobility aids with ongoing training.
  • Basic knowledge of safe food handling practices.
  • Strong verbal and written communication abilities; able to convey information clearly and professionally to staff, participants, families, and external partners.
  • Builds positive relationships with diverse populations, including aging and dependent adults, families, caregivers, staff, and community stakeholders.
  • Works effectively as part of a multidisciplinary team demonstrating sound listening skills to receive directions and understand participant concerns; maintains a cooperative and supportive attitude.

  • Basic knowledge of computers and web-based applications such as timekeeping, training and email programs, as well as electronic health records.
  • The Care Partner role requires regular physical activity, including standing, walking, bending, and lifting throughout the day.
  • Employees must be able to assist participants with transfers, ambulation, and mobility support, which may involve lifting or repositioning individuals.
  • The position also involves handling food trays, performing light housekeeping tasks, and supporting therapeutic exercises.
  • Candidates should be capable of safely performing these duties and responding quickly to participant needs in a dynamic care environment.

  • Certified Nursing Assistant, Certified Home Health Aide, or Injections Certificate preferred (additional compensation may be available for certified individuals).
  • COVID-19 and Influenza (flu) vaccinations preferred.

Responsibilities

  • Welcome participants and assist with daily program preparation and serving of beverages, snacks, and meals.
  • Provide person-centered support that enhances stability, motivation, socialization, and overall well-being.
  • Facilitate one-on-one and group activities (arts and crafts, music, discussion, and other recreational activities) with varied participant population.
  • Assist participants with transfers and ambulation.
  • Monitor changes in participants’ physical and mental condition (affect, mood, motivation, and socialization levels) and report changes to the Therapeutic Services Coordinator.
  • Support general program operations, including room setup, clean up, and laundry.
  • Attend weekly operations meetings, monthly staff in-service trainings, and regular department meetings as well as any other meetings as requested.
  • Write daily notes in participant charts related to activities provided and the level of participation.
  • Participate in the process of teaching, training, and mentoring new staff, student interns and volunteers working within the program area.

  • Additional duties may be assigned, as needed, that align with the responsibilities of the position and support the vision, mission, and values of the organization.
  • Provide assistance with toileting, bathing, grooming, skin care, and other personal care services, as prescribed in the participants’ treatment plan and under the supervision of a Registered Nurse (RN).
  • Under supervision of an RN, obtains and records participant vitals such as weight and blood pressure. Report out of range readings to the RN.
  • Document personal care services provided in chart record and report any unusual situations to the nursing team.
  • Provide foot soaks to participants as directed by the nursing team and report back on any changes in skin condition of feet and legs.
  • Maintain personal care supplies and inform nursing team of the need for supplies.
  • Assist Kitchen Aide utilize proper food handling techniques to hold, modify, portion, and deliver food to participants for the snack and lunch meals.
  • Maintain safe and sanitary procedures according to program standards and policies in all areas of food service.
  • Inform Registered Dietician or Activity Supervisor of any problems with meal service, food delivery, or related areas.
  • Perform point-of-service lunch and snack count as directed.
  • Assist participants with prescribed group exercises under the direction and supervision of the Physical and Occupational Therapists and RNs.
  • Assist each participant with meeting their therapy goals as outlined in their individual plans of care.
  • Coordinate with team to identify best ways to motivate and encourage participants to improve their mobility, balance, and overall wellbeing.
  • Plan and lead group exercises and/or group cognitive activities.
  • Complete daily documentation of participant’s individual and group therapy activities and communicate with the team regarding any changes in participant care plan/needs.
