About The Position

WelbeHealth PACE helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services, focusing on vulnerable seniors with quality and compassion. Home Care Assistants are a vital part of the team, providing aid to participants in their homes and contributing to the care planning process as members of the interdisciplinary team (IDT). Reporting to the Home Care Supervisor, this role involves the safe delivery of personal care services to WelbeHealth participants in their homes, as scheduled and directed. Unlike traditional home care roles, Home Care Assistants at WelbeHealth work a stable, full-time schedule, are assigned participants to fill a full workday, and are re-assigned in cases of cancellations. They visit 2-5 patients per day, working shorter individual shifts that add up to an 8-hour workday, and receive reimbursement for travel time and mileage between participants. WelbeHealth is seeking individuals looking to start their career in the healthcare space, offering a holistic care team environment.

Requirements

  • High school diploma or equivalency
  • Current Basic Life Support (BLS)/First Aid Certification required
  • Must be registered with Home Care Registry prior to start date
  • Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter

Nice To Haves

  • Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification preferred
  • Bilingual preferred
  • CPR and First Aid certification upon hire preferred

Responsibilities

  • Provide hands-on personal care and assistance with activities of daily living (ADLs) to participants in their homes, including but not limited to bathing, dressing, grooming, dressing, toileting, and physical lifting when transferring participants
  • Assist participants with Instrumental ADLs, including but not limited to laundry, light housekeeping, shopping, and light meal preparation
  • Measure and document participants’ vital signs as instructed
  • Act as a medical escort on vehicles to medical appointments, supporting participants with safe transfer in and out of their homes
  • Assist the Rehabilitation team by carrying out home exercise programs as indicated in the care plan developed by the IDT, after sufficient training as needed
  • Provide translation services as needed
