About The Position

Family Ties Home Care has an opening for a client Thousand Oaks. 89 male with dementia who lives with daughter and her family. He has a stable mood and likes to stay active by taking walks, playing games and keeping busy. He enjoys listening to music as well. He can be a wanderer so he must be watched at all times. CG needs to prepare meals for him and keep an eye on BP to determine if he needs more or less salt in diet. Requirements

Current HCA or higher certification
Caregiving experience of 1 year or similar experience
Compassionate and caring demeanor
Excellent communication & interpersonal skills
Ability to follow the pre-set care plan and medication schedule
Ability to lift 35 pounds
Ability to assist with client's mobility as needed
Valid driver's license and reliable transportation
Ability to work the schedule as described

Responsibilities

Following a pre-set care plan
Standard ADLs (personal care, bathing, dressing, grooming, hygiene assistance)
Assist with medication reminders
Monitor and report on client's well-being
Light housekeeping, laundry, & other household daily chores as needed
Grocery shopping & errands
Cooking/meal preparation
Scheduling appointments, including doctor's
Communication with Family Ties HC management & family members/loved-ones regarding the client's daily status
Daily notes
Other duties assigned as needed
