Caregiver/Nolensville 37135 Diagnoses: Diabetes Type 2 (controlled with diet), Hearing loss with bilateral aids, Post mini stroke (Trans ischemic attack). Allergies: Food-None/Meds-Sulfa drugs Goals & Objectives: Assist Client to get out of bed upon CG arrival. Help client get into shower safely onto shower chair, then standby assist. Prepare and serve breakfast. Med reminders. Offer mental stimulation-Engage with client in some type of activity and conversation that she enjoys. Hydration reminders. Prepare and serve lunch, cleaning kitchen after. Encourage her to do some type of exercise (walking around or chair exercises) or go on walk weather permitting. Change bed linens weekly, do laundry as needed. May run errands. ADL's: Mobility: Client is fully mobile and can rise herself from a seated position to stand on her own. She walks independently in the home mostly. She can go up & down the stairs but only with close observation and assistance-not more than 1-2x per day. She may need a little help getting into & out of a car. Toileting: Ann toilets independently. She knows when she needs to go but wears depends as a precaution for an occasional/rare accident. Eating: Client feeds herself. She is not allergic . She does need food prepared for her and served and to be encouraged to eat. For breakfast she likes cereal or oatmeal with fruit and her coffee (black) Bathing/Dressing/Grooming: Client says she likes to get a bath or shower every with standby assist. She can pick out her own clothes and can dress herself. CG may be asked to take her to get her hair or nails done at a salon. Communication: Client speaks clearly and engages well. CG may have to speak a little louder for her to hear.
