About The Position

CareLinx by Sharecare operates as a nationwide caregiver registry, offering a unique approach to in-home care. Caregivers utilize the CareLinx app to set their availability, and the company's team then matches them with clients whose schedules, preferences, and care needs align. This model provides flexibility, allowing caregivers to choose between steady recurring shifts or per-diem work, maintaining control over their work hours. The company is actively seeking experienced caregivers to deliver non-clinical, in-home care to health plan members and private clients. Once onboarded, caregivers collaborate with a dedicated Care Partner team to secure suitable matches based on location, client requirements, and personal availability. The platform emphasizes consistent scheduling, active engagement through the app, and proactive communication with Care Partners for optimal performance.

Requirements

  • Minimum 6 months of caregiving experience, OR relevant education/certification (CNA, HHA, MA, or equivalent)
  • Access to a smartphone capable of running the CareLinx app (required for scheduling, documentation, and timesheets)
  • Ability to communicate in English (bilingual caregivers strongly encouraged to apply)
  • Reliable transportation to and from client homes
  • Must pass a background check
  • Completion of program-specific training prior to accepting shifts (may apply depending on assignment)
  • Medical clearance such as TB test, physical exam, or drug screening (may apply depending on assignment)
  • Valid driver's license and MVR check if accepting driving shifts (may apply depending on assignment)
  • Additional documentation may be required based on state law or client contract (may apply depending on assignment)

Responsibilities

  • Assist clients with activities of daily living (ADLs): companionship, meal prep, mobility support, bathing/grooming/dressing, medication reminders (non-clinical), and light housekeeping
  • Complete and document all care tasks accurately in the CareLinx mobile app during each shift
  • Monitor and report changes in a client’s condition or self-care abilities to the CareLinx clinical team
  • Submit timesheets on time through the CareLinx app
  • Communicate with your Care Partner regarding any case updates or client changes
  • Ensure patient privacy, dignity, and well-being in a safe and secure environment
  • Maintain client confidentiality at all times
  • Transportation and errand support
  • Hospital discharge escort — safely accompanying members home after discharge
  • Telehealth navigation — helping members connect with virtual doctor visits
