About The Position

Our interdisciplinary team works collaboratively to ensure each patient receives personalized care that promotes healing, independence, and overall well-being. We are committed to supporting our clinicians with the resources, flexibility, and professional environment needed to deliver exceptional care to the communities we serve. The Caregiver (CNA, HHA, or PCA) provides essential personal care and daily living assistance to clients in their homes. Must be available on Mondays, Wednesdays, Thursdays, Fridays and Sundays for 12 hour shift. Caregivers support client independence while ensuring comfort, safety, and dignity.
Requirements
- Available on Mondays, Wednesdays, Thursdays, Fridays and Sundays
- 5+ years experience
- CPR certification
- Compassionate and patient-centered approach
- Ability to work independently in home environments
Nice To Haves
- CNA certification preferred
Responsibilities
- Assist clients with bathing, dressing, grooming, and hygiene
- Support mobility, transfers, and positioning
- Monitor and report changes in client condition
- Assist with light housekeeping and meal preparation
- Provide companionship and emotional support
