Joshua’s House is the West Coast's first home for individuals with terminal illness who are at risk of homelessness. Overseen by YoloCares, the program extends exceptional care to those most marginalized and neglected by traditional healthcare systems—those with no place to call home. In coordination with Joshua’s House staff, volunteers, and YoloCares’ clinical staff, the Caregiver assists residents in all aspects of their end-of-life journey, including personal care, food service, housekeeping, laundry, behavior management, socialization, activities, orientation, and information/communication needs. Joshua’s House is located in South Natomas, CA and will be the primary work location. Training will be conducted at the YoloCares office in Davis, CA.

  • Assists with or provides personal care, e.g., shower, tub bath, bed bath, oral hygiene, shaving, shampooing, cleaning nails, assists to toilet, etc. as desired by a resident, directed by a specific resident’s written Plan of Care, and supervised by YoloCares’ clinical staff.
  • Provides homemaker services by cleaning resident’s room, washing resident’s personal laundry, preparing and assisting with a resident’s meal, and other homemaking duties, when the resident’s Plan of Care directs.
  • Changes resident’s clothing and bed linen as requested.
  • Increases resident’s comfort and safety by prescribed rehabilitative measures such as positioning, ambulating and assistance with transfers. Uses basic adaptive equipment with debilitated patients when the Plan of Care requires.
  • Performs routine procedures such as feeding non-compromised residents, making occupied and unoccupied beds, and cleaning resident’s equipment.
  • Demonstrates and teaches care techniques to volunteers and interns when appropriate.
  • Records vital signs when requested by the Plan of Care and/or as requested by clinical staff.
  • Follows universal infectious disease precautions when delivering all resident care (gloves, handwashing, etc).
  • Assists residents in taking self-administered oral OTC and prescription medications as directed by the clinical staff.
  • Encourages simple measures for resident’s bowel and bladder regularity.
  • Observes/monitors resident’s physical and emotional condition and reports changes to the Joshua’s House Manager and clinical staff in a timely manner.
  • In compliance with state and federal regulations and with YoloCares policies and procedures, documents all resident care, including observations of significant changes: reactions to care, symptoms observed, mental and physical status changes in the patient, and coping changes noted in residents.
  • Participates in all Joshua’s House team meetings, training sessions, and evaluation processes.
  • Other duties as assigned by supervisors.
  • Possesses effective verbal and written communication skills to effectively report and document patient care observations and facilities-related matters.
  • Ability to perform effectively under stress and when confronted with critical or unexpected events.
  • Ability to work independently and exercise sound judgement in a care setting.
  • Must have CPR certification. If not, must obtain one within 90 days if offered the position.
  • Compliance with YoloCares’ mandated reporting policies.
  • Commitment to confidentiality and protection of resident information in compliance with HIPAA.
  • Ability to successfully meet YoloCares’ background screening requirements.
  • Acknowledgment of Yolo Cares’ mandated reporting policy.
  • Familiarity with and commitment to the YoloCares “Hospice Aide Core Values”, the “Statement on the Scope and Standards of Hospice and Palliative Nursing Assistant Practice,” and “Competencies for Hospice and Palliative Nursing Assistants” developed by HPNA.
  • Experience working a NOC or graveyard shift.
  • Experience working with adults, the elderly, and or people experiencing homelessness.
  • Bilingual skills in any language are a plus!
  • Able to use Microsoft Office Outlook and Word.
  • Strong attention to detail, time management and organizational skills.
