About The Position

Easterseals NH is currently seeking an energetic, knowledgeable, flexible individual to join our team at the Aging and Disability Resource Center ((ADRC) - formerly known as ServiceLink). The ADRC educates individuals in understanding and accessing community long term supports and services, provides additional support through the Family Caregiver Program to caregivers, and assist individuals in understanding the application process for State Programs and assists with individuals who may require assistance in understanding the Medicare program. This position will work under the New Hampshire Family Caregiver Program. The individual will be responsible for developing caregiver support groups in both the Manchester and Nashua area. They will be trained in evidence-based caregiver training programs such as Powerful Tools for the Caregiver and SAVVY. Once certified under these training programs, the individual will be responsible for establishing support groups and facilitating training for caregivers throughout the year. The individual will also be responsible for supporting the caregiver specialist to include bimonthly contact with caregivers and assisting with general office support such as answering calls and providing intake and referral assistance. Hours: This is a full-time, 40 hour/week position; 8:00am to 4:30 or 8:30 to 5pm. Compensation: $21.00 per hour

Requirements

  • Bachelor’s Degree in Social Work, Education, Human Services, Sociology, Cultural Anthropology, or Behavioral Science, OR Eight (8) years’ experience working directly with programs supporting the elderly or disabled population in lieu of degree
  • Basic knowledge of community long-term care support services for the elderly and disabled population OR combination of education and experience totaling eight (8) years
  • Basic knowledge of community services such as home health agencies, assisted livings, nursing homes, and hospitals.
  • Basic knowledge of State and Federal program such as Medicaid and Medicare.
  • Strong oral, written and interpersonal skills.
  • Valid driver’s license, auto insurance, and reliable transportation.
  • Analytical and problem-solving ability.
  • Comfortable with performing home visits and community outreaches.
  • Basic level of computer skills to include Microsoft Office application suite and database programs

Responsibilities

  • Provides ongoing support and services to caregivers under the mandates of the NH Family Caregiver Support Program
  • Provides ongoing support to caregivers through support groups and trainings.
  • Provides information on and referral to community resources which may meet the needs of the caregiver
  • Acquires through conversations with the caller, an assessment of the contact/caller’s situation/needs and provides the contact/caller with assistance in referrals to long-term supports and services through telephone contacts, walks-ins, emails, home visits/community settings and multi-media.
  • Provides information and education on community long term support and services to individuals in hospitals, rehab facilities, and/or nursing homes as mandated by the contract.
  • Completes eligibility screening and provides information to assist individuals in understanding and applying for State and Federal program.
  • Completes documentation on all contacts using assigned database following all protocols/processes as mandated by the contract and in compliance with InformUSA.
  • Complete follow-ups on contacts following InformUSA protocols and contract requirements.
  • Comply with completion of reporting requirements mandated by the ADRC contract.
  • Provide Medicare information and counseling to Medicare beneficiaries especially during the Medicare Annual Enrollment Period.
  • Attends meetings as mandated by contract or required by State.
  • Assists with general office support including by not limited to answering phones, assisting, walk-ins and other office tasks as needed.
  • Obtain all mandatory certification as required by the contract which includes the InformUSA (Alliance for Information and Referral Certification (AIRS)); State Health Insurance Assistance Program (SHIP) Counselor within six (6) months to one (1) year of hire.
  • Perform additional duties as requested.
