Requirements

Proven experience in home & community care, assisted living, nursing homes, or long-term care environments
Knowledge of patient observation techniques and ability to recognize changes in condition or behavior
Familiarity with Alzheimer’s care, dementia care, memory care, and working with individuals with developmental disabilities or disabilities in general
CPR and First Aid certification to respond effectively in emergencies
Experience with medication administration, vital signs monitoring, and use of assistive devices such as Hoyer lifts or Sit to stand lift
Ability to perform heavy lifting safely when assisting with transfers or mobility support
Strong communication skills for collaborating with healthcare teams and family members while maintaining HIPAA compliance
Compassionate approach to caring for seniors and individuals requiring hospice or palliative care support
Ability to work independently or as part of a team in fast-paced environments like group homes or in home care settings

Responsibilities

Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support and IADL'S.
Monitor vital signs including blood pressure, pulse, temperature, and respiration; report any abnormalities promptly
Support medication reminder following proper protocols and HIPAA regulations to ensure patient confidentiality
Provide companionship and emotional support to promote mental well-being
Prepare nutritious meals tailored to individual dietary needs and assist with feeding when necessary
Maintain cleanliness by assisting with light housekeeping tasks and ensuring a hygienic environment
Observe and document patient conditions, behaviors, and responses to care plans for accurate reporting to healthcare teams
