About The Position
This position provides in-home personal care and support to clients. The Caregiver/CNA works under the supervision and in collaboration with the Care Team Manager and Client Care Manager. Key responsibilities include assisting clients with daily living needs, monitoring basic health indicators, and helping to maintain a safe and comfortable home environment.
Requirements
- Active state CNA certification
- Reliable transportation
- Strong communication and companionship

Responsibilities
- Assist with bathing, dressing, grooming, toileting, and feeding
- Support mobility: transfers, ambulation, and exercises
- Take and record vital signs and report changes in condition
- Provide companionship and emotional support
- Light housekeeping and simple meal preparation
