About The Position

Under the direct supervision of the Program Manager, the Career Pathway Navigator (CPN) is responsible for applying a person-centered approach to provide individualized and targeted supports to help individuals create and navigate their own career pathway to CIE in a way that incorporates the cultural background of the individual and their family. The CPN must demonstrate the ability to establish and implement quality programming, communicate effectively with others, evaluate existing program structures and work with others to develop and implement tools, techniques and other resources to improve consumer employment effectiveness. The CPN will have a caseload of individuals that he/she will provide services to in a 1:1 individual settings. The CPN will meet and/or exceed the goals set by the organization. As an employee, the CPN is responsible for implementing all aspects of the company’s positive behavioral program, complying with the agency’s policies and procedures, and demonstrating appropriate Employee Rules of Conduct.

Requirements

  • Must possess a valid California issued Driver’s License; personal automobile liability insurance; a vehicle available for use daily; and a driving record that meets the standards outlined in the Company’s hiring policies. Business Exclusions on automobile insurance policies are prohibited. Employee automobile insurance must include a certificate of insurance verifying their policy does not include a Business Exclusion.
  • Must be willing to travel locally up to 30% of the time.
  • Must be physically and mentally fit to work with participants who are potentially dangerous to self or others in accordance with the Company’s Physical and Mental Fitness standards; must be willing to complete a health screening physical examination that includes a drug screen and TB test; and must have the unrestricted capacity to employ Non-Violent Crisis Intervention Training for use in difficult situations.
  • Must meet state personnel licensing standards.
  • Must be able to successfully complete CPR and First Aid Certification.

  • Ability to apply basic math concepts.
  • Ability to solve practical problems and use available resources.
  • Work independently with minimum supervision.
  • Interpret and follow a variety of instructions provided verbally, written in diagram or schedule form.
  • Maintain good judgment and composure during challenging situations.
  • Establish and maintain effective working relationships with participants, staff, community service representatives and the general public.

  • Association of Community Rehabilitation Educators (ACRE) certificate with an emphasis on Customized Employment.
  • Bachelor’s-level degree with three years’ experience in the field of developmental disabilities service systems or an Associates-level degree with five years’ experience.

Responsibilities

  • Serve as the organization’s representative for CCP issues and procedures.
  • Assist in developing and coordinating new policies and procedures regarding the CCP processes.
  • Evaluate and adjust programming to ensure care, safety, treatment and training for individuals.
  • Promote individual independence and empowerment by listening to individuals served, asking questions that clarify and define individual’s needs, providing flexibility and working with program staff to develop supports to meet individual’s needs.
  • Serve and actively participate as a team member, in the development and implementation of training employment goals and objectives for individuals.
  • Provide leadership which promotes and ensures high quality services to all individuals.
  • Coordinate and monitor program schedules with staff and individuals served to ensure participation in activities identified in Individual Service Plans and employment goals.
  • Gather and update all relevant data and present information with accuracy and completeness; integrate information into ideas that are organized and in style which others can act on.
  • Initiate timely conversation with others, both co-workers and external professionals, regarding individual’s health and safety, improved services and CVTC programming.
  • Thoroughly and accurately complete program paperwork.
  • Ensure that the program environment is safe, clean and organized.
  • Provide exceptional customer service.
  • Coordinate and conduct meetings; meet with other Interdisciplinary Team members to review individual’s progress, define future goals and develop objectives consistent with the participant’s Individual Program Plan and employment goals.
  • Provide direct service to participants on an intermittent and/or temporary basis.
  • Demonstrate crisis intervention techniques which reflect participant respect and dignity in all situations.
  • Perform related duties as assigned.
  • Assists with development of a Person-Centered Career Plan (PCCP) for employment, community engagement, postsecondary education, and non-work supports to achieve CIE. Works with the Individual Program Plan (IPP) team, service coordinator and participant to include PCCP goals and measurable objectives into the IPP. Supports participants in the goal setting and monitoring of their own progress. Attends IPP and IEP planning meetings as required.
  • Provide 1:1 guidance, resources and information on career pathway options including community integration, vocational training, post-secondary education, and careers to help individuals develop their path.
  • Assist with coordination of interpreters, translation and plain language materials as needed for individuals and their representatives.
  • Assist individuals and their families in acquiring and navigating employment-related regional centers and generic services and supports to address barriers and meet their unique needs.
  • Conduct a monthly review/reassessment of progress with the individual and/or to address unmet needs. Modifies PCCP as needed and expedites any changes to the plan.
  • For individuals in secondary education, attend IEP meetings as requested, partners with regional center service coordinators to support the transition from regional center youth to adult services, partners with school district representatives to encourage paid work experience at the high school level.
  • Reports on a quarterly basis basic milestones achieved, services provided, and employment outcomes along with an annual progress report/ Shares information with service coordinator regarding and needed updates to the IPP or referrals to meet person’s employment goals.
  • Assists individuals to obtain personalized benefits planning and tools for financial empowerment.
  • Works with the service coordinator to assist in the transition to ongoing induvial supported employment services or requests and extension of CCP based on individual needs.
