  • Provide customized case management interventions for young adults using Motivational Interviewing and other evidence-based practices.
  • Conduct bi-weekly one-on-ones to connect members to career development resources including internships and support services.
  • Conduct intake assessments, goal planning, and employment assessments while monitoring progress with regular advisement sessions.
  • Assist in building the Individualized Service Strategy for each youth and prepare them for follow-up services.
  • Collaborate with the Mental Health Specialist to maintain a strong network of social support partners.
  • Plan and facilitate work readiness training on topics such as resume writing and interviewing.
  • Adapt educational processes to improve outcomes for struggling learners.
  • Maintain an organized and supportive classroom environment for active participation.
  • Provide regular feedback to students regarding their progress and manage the development of their portfolios.
  • Monitor class attendance and conduct check-in calls to address barriers to success.
  • Collect outcome documentation and distribute student incentives.
  • Assist with quarterly alumni event planning and implementation.
  • Maintain knowledge of the labor market to ensure effective pathways to self-sufficiency for participants.
  • Participate in bi-monthly networking events to connect alumni to professional networks.
  • Maintain detailed monthly case notes documenting participant interactions and assistance provided.
  • Participate in case conferencing with training program staff to ensure seamless service delivery.
  • Adhere to policies for maintaining a safe working environment and ensure appropriate documentation for contracts.
  • Meet reporting requirements of funders and management of CHLDC.
  • Participate in training and agency meetings as requested.
  • Demonstrate commitment to leadership development and community organizing as strategies for social change.
  • Bachelor's degree in social work or a related field required.
  • Two years of experience in education, youth services, or workforce development, with a proven track record in case management or counseling preferred.
  • Experience navigating city systems such as public assistance, child welfare, education, and housing required.
  • Intermediate to advanced proficiency with word processing, spreadsheet, and database applications required.
  • Excellent written and verbal communication and interpersonal skills.
  • Training facilitation experience required.
  • Bilingual (Spanish) preferred.
