Requirements

  • Licensed Clinical Social Worker (LCSW) or Clinical Psychologist or RN in New York State – required.
  • Minimum 5 years of experience in hospital-based behavioral health care, with at least 2 years in a supervisory or leadership role.
  • Strong knowledge of inpatient psychiatric workflows, care coordination, and crisis intervention.
  • Demonstrate understanding of behavioral health admission criteria, utilization management principles, discharge planning processes, and payer requirements
  • Experience working with community based agencies and government agencies.
  • Familiarity with regulatory guidelines, including OMH, CMS, and Joint Commission standards.

Responsibilities

  • Collaborate with CPEP and Inpatient Unit Nurse Managers and Unit Chiefs to streamline care transitions and discharge planning.
  • Provide direct supervision to inpatient and CPEP social workers, discharge planners, and RN care coordinators.
  • Serve as a clinical advisor for complex discharge planning, social admissions, psychosocial escalations, and patient flow concerns.
  • Offer clinical supervision and guidance on crisis management, including escalations to ACS, APS, or other governmental bodies.
  • Perform regular chart reviews to ensure compliance, quality documentation, and utilization management.
  • Coordinate with external community providers to facilitate appropriate placements and step-down care.
  • Assist in recruitment, training, mentoring, and performance management of staff.
  • Act as a liaison between Behavioral Health and Care Coordination teams to ensure alignment of services and priorities.
  • Monitor and support performance metrics including length of stay, readmissions, and patient throughput.
  • Support AVP with departmental goals and special projects including audits and performance improvement initiatives.
  • Other duties as assigned.
