About The Position

The Care Transition Liaison (CTL) serves as a vital resource for patients referred to home health or hospice services. This role facilitates the transition of patients from referring physicians or hospitals to the LHC agency, ensuring that patient needs are met and that they are educated about home health and hospice practices. The CTL collaborates with healthcare professionals to coordinate care and support the admission process, acting as a liaison between various parties involved in patient care.

Requirements

  • Prior hospital, physician's office, home health, or hospice experience preferred.
  • LPN/LVN or licensed medical professional preferred.
  • Current Driver's license and vehicle insurance.
  • Access to a dependable vehicle or public transportation.

  • Experience in home health or hospice care.

Responsibilities

  • Facilitate referral of new/existing patients to home health agency following Right of Choice.
  • Collaborate with Physician and/or Case Manager to identify referred patient's needs and obtain admission approval from LHC agency.
  • Obtain and send appropriate medical record information to the agency.
  • Coordinate start of care between hospital/Physician and LHC agency.
  • Collaborate with appropriate Case Manager and/or Physician for completion of order sets needed for admission.
  • Document activities and productivity within available systems including Marketscape CRM and/or HomeCare HomeBase.
  • Assist with processing requests for medication refills for patients admitted to LHC Agency, when applicable.
  • Communicate any complaints/concerns from the Physician office/Host Hospital to the Executive Director.
