Serving clients across rural communities in Southwest Iowa, the Care Team Specialist plays a key role in improving access to care in areas where services are limited and relationships matter. The position works closely with Zion staff and Heartland Family Service to strengthen coordination, improve data sharing, and support continuity of care. This role operates within a flexible work environment that values collaboration, adaptability, and responsiveness to client and community needs while strengthening the rural mental health care continuum. The Care Team Specialist serves as a central coordination and support role, ensuring clear communication, timely documentation, and effective collaboration between Heartland Family Service and Zion. This position supports high quality, client centered care by tracking required assessments, coordinating services, and managing data and reporting requirements tied to the Certified Community Behavioral Health Clinic (CCBHC) model.
