About The Position

The care team partner will support patient care in a variety of ways including patient transfers, ambulation, and personal hygiene. This role will respond to call lights, and patient request as directed by the primary care team. The Care Team Partner will also perform tasks that support efficient operations including stocking supplies, emptying trash and cleaning equipment. Working under the supervision of the licensed nursing staff, the care partner will ensure comfort, dignity and safety while supporting infection prevention and skin integrity protocol. They will also possess the ability to exercise safe judgement and delivery exceptional patient care per guidelines established by the department to ensure optimal patient safety and patient satisfaction.

Requirements

  • 16 years of age or older Required
  • Ability to communicate effectively with patients, families, and healthcare team members.
  • Physical ability to lift, move, and assist patients safely.
  • Strong interpersonal skills and ability to follow instructions.

Nice To Haves

  • High School Diploma / GED Preferred
  • 1 year Previous experience in patient care or hygiene assistance in a healthcare setting.
  • Certified Nurse Aide - KDADS Training may be provided upon hire.
  • First Responder - RQI required within 90 days

Responsibilities

  • Respond to call lights and patient request made by patient or directed by primary care team. Patient needs may include refill water pitcher, fresh linens, coffee or other drink, warm blankets, clean up room etc
  • Respond to mobility-related requests from nursing units in a timely manner.
  • Assists with lifting and movement of patients to chairs, gurneys, beds, etc, as directed by the nurse.
  • Provide bed baths, showers, and other hygiene-related care as directed by nursing staff.
  • Assist patient in grooming task such as oral care, hair care and shaving.
  • Transport patient to and from locations as directed by care team
  • Promote infection prevention through use of standard precautions, proper procedure in dressing changes, wound care, hand hygiene and cleanliness of the patient rooms/department
  • Maintain cleanliness and organization of bathing equipment and supplies.
  • Follow hospital protocols for patient safety, including fall prevention and mobility assistance.
  • Support nursing staff with non-clinical task during high volume periods
  • Participate in departmental meetings and training sessions.
  • Provide emotional support and reassurance to patients
  • Collaborate with nursing and therapy teams to assess mobility needs and goals.
  • Educate patients and families on safe movement techniques when appropriate.
  • Complies with all policies, standards, mandatory training and requirements of Stormont Vail Health
  • Performs other duties as assigned
