About The Position

Requirements

• Registered Nurse (RN) license in the state of Colorado.

• Strong assessment and triage skills based on comprehensive risk assessment.

• Experience with Electronic Health Records (EHR) systems.
• Knowledge of immunization schedules and care coordination.

Responsibilities

• Participates as part of the interdisciplinary team to provide patients with appropriate services.
• Ensures coordination of services through the nursing process, including assessment, planning, facilitation, and evaluation.
• Assesses high-risk and rising-risk patients according to current policies and procedures for care management.
• Identifies and links patients to available resources and support systems.
• Provides education to patients, families, and caregivers to encourage engagement and self-management.
• Facilitates access to quality and cost-effective care for patients.
• Utilizes technology, including Electronic Health Records, for documentation and patient management.
• Maintains accurate documentation of all interactions and implements appropriate protocols for patient charts and billing practices.
• Supports clinic staff training needs and participates in peer reviews and audits.
• Promotes and demonstrates the mission, vision, and core values of Peak Vista.
