About The Position

As a Care Support Specialist , you’ll play a key supporting role in our team-based Care Delivery Model by assisting the Care Support Partner in ensuring clients, their family members and caregivers are met with compassion, efficiency, and care. This role is ideal for professionals with foundational experience in care coordination, social services, human services, or healthcare support who are eager to grow in a mission-driven environment. Working collaboratively with the Care Support Partner and the broader service team, the Care Support Specialist helps gather essential information, coordinates follow-ups, and ensures smooth transitions through our digital care system. You will contribute to a consistent and high-quality client experience by serving as a helpful, informed, and empathetic point of contact, especially during the early stages of engagement.

Requirements

  • Foundational knowledge of care coordination services or human services environments.
  • Ability to communicate clearly and compassionately, both verbally and in writing.
  • Basic understanding of health-related services, benefits, or social service supports is a plus.
  • Willingness to learn and apply trauma-informed and culturally responsive approaches.
  • Strong organizational skills and attention to detail.
  • Comfortable working in digital platforms, CRMs, and communication tools.
  • Ability to work collaboratively and take direction in a team-based virtual setting.
  • Enthusiastic about providing high-touch, high-quality service to those who have served
  • Bachelor’s degree or equivalent experience in Human Services, Healthcare Support, Customer Service, or related fields.
  • 2 years of experience in a client-facing or service support role, preferably in a healthcare, nonprofit, or social services setting.
  • Demonstrated commitment to empathy, confidentiality, and professionalism.
  • Ability to work independently while contributing to a collaborative team.

Nice To Haves

  • Familiarity with veteran services, public benefits, or healthcare systems is a plus.
  • Prior experience in a remote or contact center environment.
  • Knowledge of military culture or experience supporting military-connected individuals.

Responsibilities

  • Support the Care Support Partner in delivering coordinated services to clients and families.
  • Serve as an initial point of contact through our digital front door, responding to inquiries with empathy and professionalism.
  • Collect and input vital client information into the secure client portal and assist with documentation management.
  • Help schedule and coordinate service -related appointments, follow-ups, and connections.
  • Provide administrative and operational support to ensure timely service delivery.
  • Respond to general questions about services and escalate complex needs to the Care Support Partner as appropriate.
  • Participate in regular team meetings, training, and case huddles to support a high-functioning team environment.
  • Demonstrate adaptability and a positive attitude in supporting clients, their families and caregivers, across varying needs and circumstances.
