At Synergy Home Care, we are dedicated to delivering exceptional in-home care services with compassion, professionalism, and heart. We are seeking a versatile and motivated Care Services Coordinator & Community Liaison to both support quality care delivery in the field and strengthen our relationships within the community to grow our client base. This role bridges care supervision and outreach. The ideal candidate will oversee client care management performance and quality of care while also building and maintaining referral relationships with hospitals, case managers, senior communities, and other healthcare professionals. You'll be the face of Synergy—both inside the homes of our clients and out in the community. NOTE: This position requires participation in after-hours on-call rotation to ensure continuous support for caregivers and clients.
