About The Position

Care Planning Advisor - Waukee, IA Own the care strategy. Guide the family. Protect the outcome. This is not a coordination role. This is a care planning and decision-making role . As a Care Planning Advisor, you will lead families—not follow them. You will ask better questions than they’ve been asked before. You will challenge assumptions. You will design care that actually holds up over time. What You Own Lead in-home consultations that uncover true care needs (not just stated preferences) Design care plans that are safe, realistic, and sustainable Guide families through difficult decisions with clarity and confidence Serve as the single point of accountability for the client experience Navigate conflict objectively (family vs. care pro vs. reality) Continuously adjust care plans as needs evolve What This Role Requires You can build trust quickly with families in emotional situations You are objective under pressure —you don’t take sides without facts You ask deeper questions others avoid You are comfortable saying: “That plan won’t work long-term—here’s what will.” You balance empathy with accountability You take ownership of outcomes—not just activity This Role Is NOT For You If… You prefer task-based work over decision-making You avoid difficult conversations You wait for direction instead of leading You agree with clients to keep things comfortable Minimum Requirements 3–5+ years in care coordination, case management, or client-facing healthcare role Experience guiding families or clients through complex decisions Proven ability to manage multiple stakeholders and competing perspectives Strong communication, documentation, and follow-through discipline Compensation $55,000 – $65,000 base Additional growth path tied to performance and scope ownership Why This Role Matters We are not an hourly staffing provider. Requirements
