Care Plan Coordinator/Nurse Manager
About The Position
We are currently seeking a Full-Time Care Plan Coordinator/Nurse Manager to develop and complete patient assessments in accordance with the requirements of federal and state regulations. The candidate must complete minimum data set (MDS) assessments and create comprehensive plans of care after review of the patient's medical record and communication with direct care staff, the patient's physician, and family. Must be able to handle staffing, such as hiring and amending schedules and supervise nursing personnel. Requirements
- Valid Licensed Practical Nurse (LPN) license in the state of Missouri
