About The Position
The Care Navigator provides wraparound support to agency clients through a varied array of services, which may include case management, support groups, parenting classes, case management, and community education or awareness activities. The Care Navigator collaborates with other professionals and referral sources to support clients with relationship-focused, hope centered and trauma informed service delivery. All employees and volunteers are expected to be sensitive to our client's cultural and socioeconomic characteristics, reflect sunbeam family services core values and to perform as sunbeam's standards of excellence at all times.
