About The Position

San Diego’s St. Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare teams at St. Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. JOB SUMMARY The Care Navigator for the CMS GUIDE (Guiding an Improved Dementia Experience) program supports individuals living with dementia and their care partners on an as-needed basis. This role focuses on participant alignment, care coordination, individualized care planning, compliance with CMS GUIDE program requirements, and robust caregiver support—including education and support groups. Responsibilities include conducting home visits and maintaining regular communication as outlined by GUIDE protocols.

Requirements

Experience in care coordination, case management, or dementia care (preferred).
Strong communication skills, including experience with caregiver engagement.
Knowledge of dementia assessment and individualized care planning.
Ability to work collaboratively within an interdisciplinary team.
Strong organizational skills and attention to detail.
CPR Certification
YOU MUST BE ABLE TO CLEAR FINGERPRINTS THROUGH DOJ AND FBI IN ADDITION TO A DRUG SCREEN CLEARANCE.
