About The Position
Cheer Home Care is seeking a PT Care Navigator in San Diego. This role involves collaborating with interdisciplinary team members to develop and implement individualized care plans. The PT Care Navigator is responsible for providing continuous feedback to clients, their representatives, and other healthcare providers. They work closely with the office team, utilizing excellent communication and problem-solving skills. A key aspect of the position is delivering interventions, including individual treatment plans, aimed at enhancing clients' mobility, strength, and independence.
Requirements
- Active California Physical Therapist License
- 3 years of experience as a Physical Therapist
- Proof of Negative TB result
- Physical Screening Documentation within the last year
- Valid California drivers license and car insurance
- Willing to work one weekend a month as back-up on call
- Ability to multitask, drive, take action
- Strong Organizational & Communication Skills
- Cell phone with internet access

- Home Health experience preferred
