About The Position

The Care Navigator Level 1 position at NorthCare is designed for individuals passionate about making a meaningful impact in the lives of adults, children, and families. This role focuses on providing integrated healthcare services, promoting wellness, and empowering clients to achieve their goals through effective care management and resource access.

Requirements

  • High school diploma or GED with a minimum of 36 months of direct consumer care in the mental health and/or substance use field, or
  • Completion of 60 college credit hours with a minimum of 12 months of direct consumer care in the mental health and/or substance use field, or
  • Bachelor's or master's degree in any field with a minimum of 6 months of direct consumer care in the mental health and/or substance use field, or
  • Bachelor's or master's degree in a behavioral health related field, or
  • Current license as a registered nurse in the State of Oklahoma with documented experience in behavioral health care.
  • Minimum one year in a mental health-related field or healthcare setting preferred.
  • Proficiency in MS Word and general computer skills; excellent verbal and written communication skills; ability to work effectively in a multidisciplinary team.
  • Valid Oklahoma driver's license, current automobile liability insurance, and CPR/First Aid certifications required.

Nice To Haves

  • Experience in a mental health-related field or healthcare setting preferred.

Responsibilities

  • Develop and implement care management plans that integrate physical and behavioral health needs, ensuring continuity of care and client empowerment.
  • Assist clients in accessing health and disability benefits, securing housing, transportation, education, employment, and community resources.
  • Provide preventive interventions and education on topics such as smoking cessation, nutrition, exercise, and stress management.
  • Offer crisis intervention services, collaborating closely with management and therapists to ensure client safety and develop effective safety plans.
