The Care Navigator works collaboratively with the hospitalists, ancillary staff, and administration to provide seamless transition of patient care assignments with the goal of improving through-put and patient satisfaction. This role involves reviewing daily physician/patient assignments, assigning admissions, transfers, and consults daily. The Care Navigator is responsible for working directly with all physician shifts to coordinate patient assignments to ensure continuity of care, improved patient and physician satisfaction (both hospitalists and referring subspecialties). Additionally, this position works closely with the Patient Care Expeditor to help direct patient flow and enhance the discharge process. The Care Navigator assists with data collection related to patient assignments, admissions, and discharges, and functions as part of the hospitalist team in meeting quality metrics and benchmarks. Improved communication among physicians is facilitated, and assistance with data entry and submission of billing sheets to the coding department is also part of the role.
