About The Position

The Care Navigator/Foster Care Liaison at Centene plays a crucial role in transforming healthcare for members by developing, assessing, and coordinating care management activities tailored to individual needs. Requirements

• Bachelor's degree in Social Work, Nursing, Health Behavioral Science, or equivalent experience.
• 2 - 4 years of related experience in social services or community services, particularly with foster children.
• Current state's clinical license preferred.

Responsibilities

• Develop and coordinate care management activities based on member needs.
• Create personalized care plans/service plans for members.
• Educate members and their families on available services and benefits.
• Evaluate member needs and barriers to care, recommending appropriate resources.
• Provide psychosocial and resource support to members and caregivers.
• Coordinate between members and care provider teams to ensure timely access to services.
• Monitor progress towards care plan goals and collaborate with healthcare providers for revisions.
• Document and maintain member information and care management activities for compliance.
• Perform on-site visits to assess member needs and collaborate with providers.
• Provide education to care managers and members on healthcare options and benefits.
