About The Position

The Care Navigator Companion at Bishop Gadsden is a full-time role focused on providing compassionate support to residents, emphasizing their emotional and social well-being. This position involves promoting independence, facilitating engagement opportunities, and assisting with transportation and medical appointments, all while ensuring continuity of care through effective communication with healthcare teams and families.

Requirements

• High school diploma or equivalent preferred.
• Experience in long-term care for the elderly required.
• Valid South Carolina driver's license with no infractions in the past three years.
• Ability to stand, lift, bend, and stoop for extended periods.
• Ability to read and comprehend labels and written instructions.
• Strong communication skills with residents, staff, and supervisors.
• Ability to lift and carry up to 50 pounds.

Responsibilities

• Promote independence and mobility for residents.
• Develop person-centered engagement opportunities based on residents' interests.
• Provide transportation for residents as requested.
• Accompany residents to meals and medical appointments.
• Take notes during medical appointments as requested by residents and families.
• Support the RN Care Navigator with submitting Long Term Care policy claims.
• Communicate healthcare notes with family members and Care Navigator staff.
• Observe changes in client condition and report to supervisor immediately.
• Collaborate with multidisciplinary healthcare team to accomplish care plans.
• Document all information regarding client care accurately and timely.
