About The Position
The Care Navigator at Hill Country Mental Health and Developmental Disabilities Center plays a crucial role in bridging gaps in care for individuals with behavioral health challenges. This position is dedicated to providing continuity of care across various health services, ensuring a high-quality experience for individuals and families. The Care Navigator will work under the supervision of the Director of Care Navigation and will be involved in a multi-disciplinary Care Team to support wellness and recovery.
