Position Summary: The Care Navigator Assistant will work to support Care Navigators to support addressing identified Social Determinant of Health needs and population health gaps in care.

  • Complete SDOH screening for assigned patients
  • Connect food insecure patients to food resources in the community such as food pantries and provide information and assist with applying to SNAP (food stamps) as appropriate.
  • Connect pediatric patients to clothing resources.
  • Support Cradles to Crayons clothing packet assembly.
  • Support monthly bills workshop.
  • Support processing discount program applications for patients who attend bills workshop.
  • Support weekly food distribution at CRWC. Assist with unloading, bag assembly and food distribution.
  • Support winter coat distribution and car seat distribution
  • Assist in updating NH community resource handouts
  • Provide outreach calls and appointment scheduling to support population health efforts
  • Provide assistance at Self-Monitoring Blood Pressure clinic
  • Participate in training to further understanding of both community resources and the intersections of social justice and community health work.
  • Maintain excellent punctuality and attendance.
  • Promote a sense of “team work” through demonstration of self-direction and self-motivation.
  • Solve problems independently or knows when to seek consultation.
  • Displays outstanding customer service skills when interacting with all NeighborHealth customers according to the ACE model.
  • Adheres to all NeighborHealth and departmental policies and procedures.
  • Participate in required departmental activities and meetings.
  • Perform other related duties
  • Enrolled at Northeastern University and eligible for Coop program
  • Classwork related to public or Community Health
  • Ability to understand and speak Spanish.
