About The Position

The Care Management Coordinator - ICD is responsible for coordinating and managing care services for patients, ensuring high-quality, timely, and coordinated care, with documentation in compliance with International Classification of Diseases (ICD) coding standards. They work closely with healthcare providers in developing care plans, coordinating referrals, and ensuring patient progress. The role also involves reviewing medical records, identifying coding discrepancies, and providing education and guidance to internal and external staff on ICD coding guidelines. Strong attention to detail, work ethic, knowledge of ICD coding principles, and effective communication skills are essential for success in this role.

Requirements

  • High School Diploma or equivalent experience
  • 0-3+ years of related experience.
  • Performs work under minimal supervision.
  • Handles complex issues and problems, and refers only the most complex issues to higher-level staff.
  • Possesses comprehensive knowledge of subject matter.
  • Provides leadership, coaching, and/or mentoring to a subordinate group.
  • May act as a lead or first-level supervisor.
  • PC Skills including: Email, Word, Excel, PowerPoint

Responsibilities

  • Ensures that member needs are addressed and that members are referred to the appropriate service provider(s)
  • Administers policies and procedures for verifying member identity, membership status, and entitlement to services
  • Maintains current and accurate membership databases and records
  • Suggests new service offerings or revisions to existing programs based on member feedback or in response to member needs or gaps in coverage
  • May participate in member retention or renewal efforts
  • Other duties as assigned.
