About The Position

The Care Management Assistant is a full-time position working Monday to Friday from 9:30 AM to 6:00 PM. As a member of the multidisciplinary team, the Care Management Assistant provides discharge planning support to the Care Manager and Social Worker. The role involves supporting patients and families/caregivers by reducing non-clinical barriers to discharge in the Inpatient and Emergency Department (ED) environments.

Requirements

• High School diploma or equivalent (GED).
• Two years of experience in a healthcare or social services setting.

Nice To Haves

• Associate's degree or two years of college in human services/health related discipline preferred.
• Knowledge of community-based/post-acute care resources such as primary care, skilled nursing facilities, home health, durable medical equipment, and transportation.

Responsibilities

• Provide discharge planning support to the Care Manager and Social Worker.
• Assist patients and families/caregivers in reducing non-clinical barriers to discharge.
• Work within the Inpatient and ED environments.
