Cayuga Centers is seeking a Care Manager to provide goal-directed support to children and families through comprehensive, family-driven Plans of Care. This role involves coordinating services, conducting assessments, and ensuring families receive the necessary support to address behavioral, physical, and social health needs. The Care Manager will work closely with families and service providers to create and implement effective care plans, while also providing outreach services to eligible children in Ontario County and surrounding areas.

  • Build trusting relationships with families to ensure they have the support needed to be successful.
  • Provide Health Home Care coordination services for children with varying acuity needs, including face-to-face services.
  • Educate families and children about the nature and desired effects of services and conditions being treated.
  • Document all case activity, including outreach, consent development, assessments, and client progress.
  • Administer CANS-NY assessments at least once a year and update as needed.
  • Inventory and coordinate existing services relevant to the Plan of Care, identifying additional services as appropriate.
  • Convene yearly meetings to review progress, update Plans of Care, and confirm client eligibility.
  • Ensure responsible transitions of client services between health homes and care settings.
  • Assist families in overcoming barriers such as transportation and acquiring public benefits.
  • Bachelor's degree with a minimum of two years experience; or Master's degree with a minimum of one year experience; or Registered Nurse with a minimum of two years experience.
  • Relevant experience in serving children and families in child welfare, developmental disabilities, behavioral health, primary health care, or social services.
  • Experience coordinating and participating in team settings.
  • Ability to deliver family-driven, youth-guided services.
  • Valid NYS driver's license and personal vehicle required.
  • Experience in child welfare or social services settings.
  • Familiarity with public benefits acquisition processes.
