About The Position
This is a full-time hybrid Care Manager role located in the Fox Valley area with some remote work flexibility. The Care Manager will be responsible for case management, training, social services, and collaborating with medical professionals to assure high-quality care solutions. Will be supporting members in Appleton and Green Bay area so must be willing to commute within that range. Will spend 50% of their time meeting with members at their homes are attending doctor appointments with them, the other 50% of the time will work remotely completing additional duties.
