Abound Health NJ is seeking a Care Manager to join our dedicated team in New Jersey . In this role, you will report to the Program Director. The Care Manager is responsible for providing support, services, advocacy, and guidance to individuals receiving New Jersey Waiver Services while upholding Abound Health NJs mission, vision, and core values. You will ensure high-quality service delivery, maintain compliance, and support both clients and direct care staff.

  • Review client service/grid notes and ensure documentation meets regulatory standards
  • Maintain accurate and up-to-date client and staff records
  • Conduct clinical and supervisory meetings, annual evaluations, and staff development plans
  • Participate in staff meetings and client ISP meetings; advocate for client needs
  • Develop individualized goals, task analyses, and interventions
  • Complete assessments, progress reports, and AENs as required
  • Oversee staffing patterns, scheduling, and PTO/payroll requests
  • Ensure completion of client-specific trainings and accurate OnTarget goal entry
  • Resolve client/guardian grievances and complete incident reports within required timelines
  • Build community relationships, represent Abound Health NJ at events, and support resource access
  • Strong attention to detail, accuracy, and organization
  • Ability to manage changing priorities and work in a fast-paced environment
  • Excellent communication and interpersonal skills with clients, families, staff, and community partners
  • Ability to multitask, problem-solve, and maintain professionalism under pressure
  • Strong understanding of ISP implementation, goal writing, task analysis, and Medicaid/third-party billing
  • Ability to work collaboratively within a team and maintain therapeutic relationships
  • Alignment with Abound Health NJs mission, vision, and values
  • Masters degree + 1 year of experience working directly with persons with disabilities
  • Bachelors degree + 2 years of experience
  • Associates degree or 60 credits + 4 years of experience
  • High school diploma/GED + 5+ years of experience
  • At least 18 years of age
  • Valid drivers license and safe driving record
  • Clear background checks (Criminal, NJ State Police, FBI if required, SAM, MEDI Check, LEIE, Sex Offender Registry, Child Abuse Registry if required)
  • Reliable transportation with current auto insurance, registration, and inspection
