About The Position

Community Care is hiring a Case Manager/Care Manager to join their Family Care Program team. This full-time opportunity offers a weekday schedule and a $5,000 SIGN-ON BONUS. The position is based in Kenosha county. Community Care is a local nonprofit with more than 40 years of experience helping older adults and adults with disabilities live as independently as possible within the community. Their mission is to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. They coordinate and deliver a full range of supportive services that help more than 13,000 Wisconsin residents live safely, confidently, and with dignity. Community Care is Wisconsin-based with local offices supporting each of the 23 counties they serve. Their dedicated team works with members to develop care plans to meet their health and social needs, committed to providing compassionate, personalized care that empowers members to live a healthy and independent lifestyle.

Requirements

  • Bachelor’s Degree in a Human Services or closely related area required. A minimum of three (3) years’ experience working with seniors or adults with physical and/or intellectual disabilities may be substituted for a non-human services degree.
  • Eligibility for long-term care functional screener certification is required
  • Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Nice To Haves

  • One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity preferred.

Responsibilities

  • Manages an assigned caseload of program members in collaboration with an RN Care Manager
  • Completes comprehensive member assessments, member centered care plans, & functional screens
  • Provides ongoing care coordination and quality assurance monitoring of services
  • Maintains required documentation in adherence to established standards.
