About The Position

Coordinate and manage care for elderly and adults with physical, developmental, and intellectual disabilities who are members of the Community Care, Inc. program. DUTIES: Perform initial and ongoing assessments to include care plan development, risk reduction strategies, and service coordination. Identify, evaluate, implement, and monitor long-term healthcare needs and manage services. Maintain clear and updated documentation of all contacts and services provided. Perform home visits and maintain a regular schedule of client contacts and visits. Provide access to community resources for clients and families. Maintain contact with a family member on needs and services offered to a client. Contact service providers to coordinate and authorize services to meet the identified needs of clients

Requirements

  • Must have excellent interpersonal and communication (written and spoken) skills and the ability to work in a multicultural environment.
  • Exceptional organizational and multitasking skills.
  • Must be dependable, flexible, detail-oriented, and have basic computer knowledge.
  • Bachelor’s degree in social work, Human Services, Psychology, or another related field, and a valid driver’s license and insured vehicle.
  • At least one (1) year of formal (non-family setting) experience working with a geriatric population in community care management or related community experience with older adults.

Nice To Haves

  • Previous experience working with the elderly and disabled is seen as an advantage.
  • A bilingual Spanish speaker is highly preferred but not required.
