About The Position
Piedmont Macon North Hospital is a 103-bed hospital located in North Macon. Highlighted by a robust emergency department and a strong and growing elective surgical program, we offer expertise in a variety of specialties, including bariatric and orthopedic care, gastric reflux disease, and kidney stone treatment. The Care Manager is responsible for care coordination, progression of care, and proactive discharge planning and is accountable for expediting the timely and safe discharge for all patients in their case load.
Requirements
- Graduate of Nursing Program Required
- 2 years of experience in an acute or post-acute setting Required
- Experience using InterQual or Milliman Preferred
- RN - Registered Nurse - Georgia State Licensure and/or NLC/eNCL Multistate Licensure Required
Nice To Haves
- Master’s degree Preferred
- Certification in Case or Care Management Preferred
