Requirements
- Strong interpersonal and communication skills
- Experience in care management or similar role
- Empathy and understanding of the needs of seniors and their families
- Ability to effectively coordinate and manage multiple tasks
- Proficient in using care management software and technology
- Valid driver’s license and reliable transportation
Responsibilities
- Collaborate with clients, families, and healthcare professionals to develop customized care plans.
- Conduct assessments to determine individual client needs and preferences.
- Coordinate and schedule caregiving services based on client requirements.
- Supervise and train caregivers to ensure quality care delivery.
- Regularly perform client visits to monitor care quality and address any concerns.
- Maintain accurate and up-to-date client records and documentation.
- Act as a liaison between clients, families, and healthcare providers to facilitate effective communication and optimal care management.
- Stay updated with industry trends and best practices in care management.
