About The Position
Baptist Medical Center is looking for an Care Manager to join our team in Jacksonville, FL. This is a Full-Time days opportunity located at Baptist Metro Square campus. As an Care Manager, you will be responsible for: High level screening, assessing and planning for an identified high risk group of patients in the ER, Critical Care and other services as needed. Collborate with the attending, ancillary, professional and technical staff to coordinate patient care. Screen patient for admissions or discharge utilizing Utilization Review criteria. Requirements
- 3-5 Years Case Management Experience
- 3-5 Years Emergency Department Experience
- 3-5 Years Discharge Planning Experience
- 3-5 Years Critical Care Experience
- Basic Life Support (BLS) Required
- Licensed Registered Nurse Required
- Bachelor's Degree - Nursing

- Master's Degree - Nursing
